4x1 – Episode 1
Auschlander and Craig have trouble dealing with the resignation of Westphall; a mentally ill amnesia patient gets mistaken for the chairman of the Residency Review Committee; Chandler and Morrison have a falling out over Vietnam.

Luther goes out on his first day of paramedic training; Craig is reunited with his estranged son and new wife; Westphall alienates the hospital staff after being back for only one day.

4x3 – Episode 3
The Craigs deal with the aftermath of Stephen's death; Caldwell's excursion to a rented cabin on the cape is crashed by Fiscus; Axelrod becomes self-conscious about his weight.

4x4 – Episode 4
The Craigs prepare for Stephen's funeral; Caldwell's sexual indiscretions lead to a practical joke being played on him; Westphall tries to sell the hospital board on increasing funding for malnutrition; Luther agrees to help Axelrod lose weight.

4x5 – Episode 5
The Craigs meet Yvonne's parents and their relationship gets off to an icy start; Westphall's efforts to start a hunger unit in the hospital takes a toll on his health; Caldwell's sexual escapades take a scary turn; a cancer patient's impotence following an operation causes Fiscus to reflect on his own life.

4x6 – Episode 6
Westphall is visited by his daughter at the hospital; the Residency Review Committee puts pressure on Caldwell and Turner to perform more surgeries; Axelrod's inability to lose weight costs him a relationship; two mentally challenged patients are caught having sexual intercourse; an astronaut is treated for an unknown condition which causes paralysis in his legs.

4x7 – Episode 7
Craig travels to Clinton, Ohio in hopes of bringing back his granddaughter. Ehrlich clashes with Caldwell over an unnecessary plastic surgery. Morrison goes against the US military by refusing to discharge an astronaut. John Doe #6 takes the identity of Mary Richards from The Mary Tyler Moore Show (1970).

4x8 – Episode 8
Craig gets a surprise visitor with good news; Ehrlich is demoted to gynecology; Westphall treats a patient who is suffering from malnutrition; Morrison takes the Remedial Exam for the third time; the tension between Rosenthal and Papandrao hits a boiling point; Chandler's romance with Turner faces a major hurdle.

4x9 – Episode 9
Fiscus is reunited with a homeless man that once helped him move; Ehrlich helps deliver a baby; Westphall's housekeeper is admitted to the hospital with a back injury; Caldwell steps over the line with Papandrao.

4x10 – Episode 10
There is a citywide blackout in Boston and St. Eligius is hit hard. The so-called "back up" generator does not start automatically and this endangers surgery. Therefore, the Drs. Auschlander and Craig decide to turn off power for all "unnecessary" parts of the hospital, including the elevators. However, Dr. Westphall is stuck in the elevator with a character; Dr. Fiscus proposes to his lady love and she accepts.

4x11 – Episode 11
Santa Claus comes to the hospital and collapses while meeting the children; the Craig hosts a Christmas party; Ehrlich is upset when no one buys him any gifts; Westphall has Lizzy and her boyfriend over for the holidays.

4x12 – Episode 12
Dr. Chandler decides to do his volunteer work at the Boston Women's Clinic where protesters caused criminal damage and were impeding the operation of the clinic. The young mentally challenged girl, Pru, is scheduled for an abortion at St. Eligius; Dr. Morrison is unwilling to perform his job as her attending physician because of his personal beliefs. Luckily, Dr. Erlich is set to perform the abortion. A bomb explodes at Boston Women's Clinic; no one knows if Dr. Chandler survived. 4x13 – Episode 13
A bomb is planted near the hospital's gift shop; Fiscus is unhappy when he is forced to do his community service with the elderly; Rosenthal must deal with Papandrao both at work and in the courtroom; John Doe is close to discovering his true identity; Ehrlich and Chandler differ over how to treat a couple that is struggling to conceive.

4x14 – Episode 14
The matriarch of the wealthy political Endicott family comes to the hospital for an extended stay; Auschlander is less than thrilled about the Endicotts being at the hospital; Morrison must undergo surgery for the first time; the Valeres finally make a decision on the future of their family; after making an embarrassing mistake on one of his patients, Axelrod is asked to treat a gorilla; Fiscus has second thoughts about his engagement to Mona.

4x15 – Episode 15
After giving blood, Caldwell gets some sobering news. Mrs. Endicott decides to go under the knife. Morrison, Fiscus, Clancy and Mona's love lives become entangled.

4x16 – Episode 16
The Endicotts deal with the aftermath of traumatic events. Caldwell struggles to come to grips with his prognosis. Ehrlich meets a teen with growth problems while doing his community service. Fiscus is leery about taking his relationship with Clancy to the next level.

4x17 – Episode 17
St. Eligius' 50th anniversary spurs memories, including its opening in 1935 under the leadership of Father Joseph McCabe, the arrival of recently emigrated Helen Rosenthal and Mark's return as chief of surgery, Mark's bypass on Patrick O'Casey and the death of Maureen Westphall. Morrison struggles to diagnose a difficult patient and cope with the loss of Pete to a kidnapper.

4x18 – Episode 18
Morrison still tries to find out what is wrong with Terence O'Casey and is frantic as to where his son is. In 1935, Westphall's entire family, with the exception of his father, is killed in an accident. In 1945, Westphall was so unruly that Father McCabe resorts to an unusual tactic to straighten him out. Auschlander arrives and has to deal with anti-Semitism and meets the woman whom he would later marry. O'Casey's parents come to visit their three-year-old daughter, who is sick. 4x19 – Episode 19
Axelrod and Fiscus are sent to a medical conference in California; Ehrlich treats a football player will do anything to get another chance to play; Morrison must tell a child that one of his arms will need to be amputated; the Craigs seek counseling; Novino's first day on the job as a doctor does not go very well for her.

4x20 – Episode 20
Alex Corey is brought back to the hospital to paint a portrait; Chandler does his community service at a crisis hot line and gets a suicidal caller; the Craigs' marriage continues to show strain; Axelrod struggles for self respect; Westphall invites Novino over for dinner.

4x21 – Episode 21
Novino treats a cartoonist with a life-threatening condition; Westphall asks Rosenthal for advice on his feelings for Novino; Morrison performs his community service at a prison; Ehrlich must treat Mrs. Craig and is very leery about diagnosing her with anything for fear that it will jeopardize his working relationship with Dr. Craig.

4x22 – Episode 22
Turner returns from Mississippi; Chandler is nervous about the status of his relationship with Turner; Auschlander ends up being in the wrong place at the wrong time when he ventures to a property that is owned by the hospital; Craig becomes more irritable after temporarily giving up custody of his granddaughter.

4x23 – Episode 23
Westphall and Auschlander don't see eye to eye on plans for an adjoining property; Turner gets attached to an orphaned baby; Luther fears that he might never be more than an orderly; Rosenthal is visited by her reformed son and first husband; Novino gets plenty of male attention at the hospital; Ehrlich joins Chandler at a protest rally; the Craigs argue about their granddaughter's baptism.

4x24 – Episode 24
A mentally-disturbed man who thinks that he is the city of Boston roams around the ER. Novino accompanies Westphall to the Doctor of the Year banquet. Craig has an auto breakdown which causes him to miss the awards ceremony. Ehrlich is torn when he gets an offer from Boston General. Axelrod tries being less amenable to other people; the third year residency list is released.
